In Family Medicine we provide continuing and comprehensive health care for the whole family, from infants to seniors. We are a residency program, which means our providers educate graduate physicians to become specialists in Family Medicine.

Our providers are dedicated to helping patients stay healthy and offer a full range of healthcare services.

What should I bring for my first appointment?

For your first appointment, bring your current prescription information or medical records from non-Temple Health System facilities. Complete a medical records release form to authorize the transfer of health records from another healthcare facility to us. What are my payment options, and do you offer financial assistance?

Visit the billing page of your preferred location for information on insurance, billing and payments.

We serve patients in difficult financial circumstances and offer financial assistance to those who have an established need to receive medically necessary services. Learn more about financial assistance options.

Do you have an after-hours number in case of emergency?

Cancellation/No Show Policy

Please give 24 hours’ notice for all cancellations.

How to Obtain a Referral or Prescription Renewal

Patients can request referrals or prescription renewals through myTemple Health or by calling the office. Referrals require 72 hours’ (3 days) notice. Please allow the office 24 hours (1 day) before pick up. Prescription renewals will be refilled within 24 hours. See our staff for my Temple Health enrollment information.
