Programs Supporting Social Determinants of Health

Available to Temple Care Integrated Network Providers

Health is shaped by a variety of factors, from nutrition to safe housing to social connection; yet many people in our shared patient community lack secure access to these foundational requirements. TCIN providers can be instrumental in connecting patients with services; Temple is committed to providing you with resources to help your patients access the building blocks of well-being.

Food as Medicine: Farm-to-Families

Diet and nutrition are key to wellness, especially in populations with a high prevalence of chronic disease. A partnership with St. Christopher’s Foundation for Children and Lancaster Farm Fresh Cooperative, the Farm-to-Families program delivers fresh produce to community hubs including Temple University Hospital, where it is sold at affordable prices and can be purchased with food stamps. In addition, physicians who screen patients for nutrition access can “prescribe” coupons for them to receive fresh produce through the program. Please follow this process to prescribe fresh produce for your patients:

  1. This program is recommended for patients who screen positive for food insecurity and/or do not have access to fresh produce.
  2. Patients must reside in 19121, 19132, 19133 or 19140
  3. The patient must have Health Partners Plans insurance
  4. There is no prescription required. Providers may submit a digital referral

Neurorestoration Workshops

The Frazier Family Coalition for Stroke Education and Prevention offers workshops for patients and their friends and families after a stroke.

September 20, 2022: Tips to Prevent Strokes and Help the Body and Brain Keep Active

Learn steps to keep your brain and body healthy and decrease the risk of having a stroke. We will go over tips about exercise, healthy nutrition, sleep, and keeping engaged in cognitive and social activities.

October 18, 2022: Options to Boost Brain Health After Stroke

We will talk about fundamental principles of exercise, healthy nutrition, healthy sleep hygiene, and explain how to link these to post-stroke care including occupational and physical therapy, psychotherapy and mind-body practices such as yoga, meditation, and prayer.

November 15, 2022: Transforming Function After Stroke

This talk will give an overview of the new options available in 2022 and also the current clinical trials happening in the Philadelphia region. We also talk about new approaches currently being tested in laboratories in the Philadelphia region that could allow doctors to one day literally rebuild the brain after a stroke.

Comcast offers Internet Basics courses that you can share with your patients who are not yet comfortable navigating the web.
