Temple Care ACO REACH: Information for Patients

Temple patients with Medicare who have joined the Temple Care ACO REACH program have access to additional benefits including easier access to a skilled nursing facility and short term skilled home health (if you’re not homebound), home visits after you’ve been discharged from your hospital stay.

Patients may of course choose from any Medicare enrolled skilled nursing facility or home health agency but the waivers only apply when selecting from the list of our partners. Please see our list of preferred Skilled Nursing Facilities and Home Health providers.
